United Hospice respects the rights of patients to make choices about their health care.
An advance directive gives patients a way to make their wishes known so that they are sure to receive the kind of care they desire. In addition, completing advance directives help you feel reassured that your choices can be honored immediately, without losing valuable time, and those difficult decisions will be easier for your loved ones to make.
Advance directives consist of four types. Each enables you to convey your end-of-life wishes if you are unable to communicate. They include:
- A Health Care Proxy
- A Living Will
- A Do Not Resuscitate Order or DNR
- An Organ Donor Designation.
The best time to write advance directives and make your wishes known is while you are healthy.
