NYS Licensed Speech Language Pathologist for Rockland & Orange Counties, Per Diem

Title: Speech Language Pathologist
Location: Rockland & Orange Counties
Schedule: Per Diem, as needed
May Require Weekend Work as Needed
Job Duties:

Conducts initial assessment and evaluation of patient’s level of functioning relating to speech, language and swallowing ability: follow-up visits as needed per MD order

Identifies problems and develops therapy care plan according to patient’s needs with adjustments to care plan as appropriate

Reviews care plan with IDT; participates in IDT meeting as possible

Directs and assists patients and families on interventions for swallowing, speech and communication needs; evaluate family’s ability to assist patient

Participates QAPI program and hospice sponsored in-service training

Observes records, reports to family, nursing , physician and other IDT members about patient’s response to treatments and any changes in patient’s status

Accepts other responsibilities as assigned

Ability to provide direct care according to the established patient plan of care and delegate to others

Incorporates organization’s commitment to customer service and continuous quality improvement into daily work habits
Qualifications:
- Holds a current New York State license in speech pathology
- Commitment to the Hospice philosophy of care
- Ability to relate to a diverse population and workforce in a personable and effective manner
- Must be proficient in Microsoft Office, internet and EMR
This position is per diem/as needed position and is not benefits eligible.
