Performance Improvement Manager

Full time, Exempt, Benefit-Eligible Position

United Hospice has an exciting new opportunity for a full time Performance Improvement Coordinator to support our growing organization. We have just begun our search for a highly skilled, experienced individual to join our team. In this critical role you will manage compliance with Hospice Item Set (HIS), Joint Commission, Centers for Medicare and Medicaid (CMS) and NYS Department of Health regulations.  

As an established and highly regarded non-profit hospice in Rockland County, we are now expanding our services to include Orange County.  We are seeking someone with a passion for end of life care, compliance, education and excellence. 

As Performance Improvement Manager you will be an essential member of both the Administrative and Clinical teams, serving as gatekeeper of our annual Performance Improvement Plan (PI). You will manage compliance with Clinical Record reviews, Emergency Preparedness Plan and Quality Assurance-Risk Management.  

As one of your essential job responsibilities, you will Chair our Professional Advisory Committee (PAC), facilitating quarterly meetings to review incidents, policies and internal protocol. You must be well versed with Joint Commission accreditation and Hospice Assessment of Health Providers and Services (CAHPS). You are responsible for OSHA reporting and Corporate Compliance. Experience with submission of HIS data is strongly preferred. Hospice and PI experience is essential. Registered Nurse (RN) with an active NYS License and BSN required, Master’s in Nursing or Public Health preferred.

You are adept at creating engaging presentations and educational seminars and at ease presenting materials to large groups. You are a passionate advocate of professional development and compliance.

You must be proficient in Microsoft Office applications, CMS, Joint Commission and electronic medical records (EMRs). Your schedule is flexible with occasional weekend and evening hours as needed as you will participate in Administrative On-Call (AOC) responsibilities with clinical team rotation.
