United Hospice has new and exciting opportunities for compassionate and experienced bi-lingual Hospice Social Workers to join our talented team. As we prepare to expand our services to Orange County, we are seeking a full-time, bilingual, Social Worker to work with our team, brining hospice care to patients and their loved ones in Orange County, NY. This new and exciting full-time position provides a unique opportunity to join a talented team providing compassionate care to hospice patients and their families.

In this critical role, you will represent the heart of our strategic initiative as we reach out to our underserved Latino Communities to build trust, establish relationships, and create a pipeline of referrals, as we strive to break down traditional cultural and language barriers that prevent families from seeking and receiving Hospice services. You understand the Latino Community and you are passionate about Hospice services and adept at networking. You will become a trusted advisor and ultimate liaison between our Latino communities and hospice services.

It is essential our employees have an understanding and commitment to hospice philosophy of care. Working with a diverse patient population, you must be able to communicate effectively, while sustaining a caring, kind and professional disposition. The successful candidate will have strong clinical assessment skills and excellent written and verbal communication.

You must possess the ability to accurately assess the psychosocial needs of patients admitted to our program. You will be at ease relating to a culturally diverse population while being adept at building a rapport with the terminally ill patient and their family/loved ones.

The qualified candidate must have at least two years supervised direct practice experience in a health care setting or prior experience with post-acute hospital services. Hospice, end-of-life care experience preferred. Weekend hours required.

Daily access to an automobile is required. A valid NY or NJ State Driver’s License, ability to drive and driving record acceptable for agency insurance coverage are required. If you are working in our community-based program, you will be driving from patient residence to patient residence with mileage reimbursement for travel incurred.
