Admissions and Business Development Nurse (RN), Hospice, F/T

United Hospice has a new and exciting opportunity for a compassionate and experienced Admissions and Business Development Nurse with Hospice experience, to join our talented team. As we prepare to expand our services, we are seeking a Rockland County-based Admissions and Business Development Nurse. Must be a NYS licensed RN.

The successful candidate will have strong clinical assessment skills, business development skills as well as excellent written and verbal communication.

As Admissions and Business Development Nurse, you will serve in an exciting dual role to expand United Hospice’s reach while also having time to provide clinical care by conducting the initial visit with patients and their families in their home, inpatient facility or in our United Hospice office to describe our comprehensive services. This will include but is not limited to explaining Hospices’ philosophy, services, and Advance Directives. You will assess the patient and family’s needs for an array of hospice services. You provide patient family counseling and education when appropriate.

The position is responsible for promoting awareness of United Hospice in assigned local markets (metro area including a specific focus on Rockland and Orange Counties), with a strong focus on educating and developing cooperative working relationships with prominent provider groups. This will require extensive local market research, sustained outreach to the leadership of provider groups, active involvement in community and healthcare organizations, organizing events and the ability to deliver compelling presentations and messaging about United Hospice's service value, mission, and reputation. It will also require collaboration with other marketing staff to design and execute marketing tactics such as social media targeted at caregivers. The position will provide admission support when needed. The primary responsibility of the role is to educate referral entities on Hospice eligibility and admission process. Is available to provide education to medical groups and execute on the spot admission assessment if requested. The position is a dual role blending the skills of an Admission Nurse and Business Development professional.

 

Position will lead public outreach process to prominent provider groups with Director of Marketing in assigned markets, including research to identify key contacts, prioritization of targets and building of detailed business development plans. Execute outreach plans through combination of personal calling with mail, e-mail, and call center-based tactics such as follow-up to United Hospice Intake Center leads. Executes Hospice eligibility and admission educational forums at medical groups or as requested. Collaborates with Director of Nursing to target medical groups referring patients to United Hospice and to generate referral relationships with practices requiring strengthening. Effectively represent United Hospice services generates productive dialogues with the leadership of prominent provider groups, emphasizing continuity of care for their seriously ill senior patients and gaps in care that United Hospice can address with high clinical quality. Constantly educates medical providers in the community on the value of Hospice. Breaks down the myths regarding Hospice care and converts providers into collaborators with United Hospice. Work directly with local hospitals and UH Hospital Liaisons to represent United Hospice services and benefits to referrals upstream during end of life care. Increases referrals each billing cycle. Increases length of service.  Collaborate with marketing staff and field clinical /operations leadership to move through the engagement process. Oversee preparation for meetings and ensure timely follow-up to address questions, build trust and initiate joint patient communications and processes. Coordinate presentations requiring execution by United Hospice Medical Director and or Director of Clinical Services. Role may vary from lead presenter to providing support for clinical leadership, depending upon the needs/potential of the specific provider group. Responsible for leveraging the time of United Hospice clinical leadership for maximum impact. Executes necessary follow-up on behalf of leadership. Ongoing account management for key provider groups once relationships are established, with a strong customer service ethic. Tracks referral production from each provider group. Drive regional brand awareness of United Hospice in concert with marketing staff and agency resources.

 

Admission responsibilities consists of performing physical and pain assessments, obtaining demographic, medical and historical information. This includes obtaining all necessary consents and authorizations to prepare the initial nursing care plan. For patients who require an admission while inpatient, you will work with the hospital discharge planning team to effectively coordinate a seamless discharge to hospice. It is anticipated that 20 percent of time with be involved with direct admission services and 80 percent will be dedicated to generating referrals to United Hospice.

Produce business plans and strategies for new business development opportunities. Generate annual development business plan. Support overall business development activities for organization.  Participate in constant business development efforts. Develop business development strategy supporting United Hospice growth objectives. Identify new business opportunities in assigned business markets. Ensure business development objectives are aligned with UH strategic business development initiatives.  Manage the business development pipeline.  Implement growth-oriented business development and marketing objectives in United Hospice plans.  Lead market assessment of business opportunities both existing business operations and external business opportunities. Develop a business plan to maximize new business opportunities.  Develop and implement business development strategy for growth. Develop local business development strategy and goals.

 

Qualifications:

Registered Professional Nurse (RN) with a current NYS License; The qualified candidate must have at least two years recent clinical nursing experience in either hospice or hospital-based med/surg nursing, with electronic medical record competence. Direct experience in healthcare sales and community education is required. Admission and end-of-life care experience preferred; experience in home care, hospice and/or oncology preferred; and bi-lingual Spanish-English a plus with additional compensation at hire.
